There is increasing evidence that some men go through hormonal changes as they age. The most common hormone attributed to these male hormonal changes, often called male menopause, is testosterone. Most of the testosterone (98%) is bound to proteins in plasma, leaving only a small amount of free hormone. Testosterone production is regulated by gonadotropin releasing hormone (GnRH) & luteinizing hormone (LH) in a negative feedback system.

The loss of testosterone may be because of 3 general reasons:

The number and volume of leydig cells in the testicles decrease with age at about 1% per year after the age of 50 years.
Aging leads to loss of the LH pulse amplitude as well as a decreased response of LH to GnRH.
Increase in SHBG lead to a further decrease in free testosterone.

Other factors may also play a role in the levels of testosterone in men, like genetic composition, alcohol (which is known to suppress testosterone by testicular cytotoxicity), decrease blood flow to the testicles and smoking. Obesity itself may play both a causative and effectual role in decreased testosterone levels.

The effects of ageing on testosterone levels may be confounded by other variables that must be considered in the assessment of androgen status. Older men recruited from medical clinics have lower testosterone levels than community-dwelling populations, suggesting that concurrent ill health depresses testosterone levels.

Chronic illness, prescription medication, obesity or excessive alcohol has been associated with a substantial reduction in testosterone levels. A direct effect on serum testosterone levels may be seen with medications such as opiates (change in LH pulse amplitude) or anticonvulsants (hepatic enzyme induction). Surprisingly, various lung disorders have been found to reduce testosterone levels irrespective of age, the effect being more marked in those with lung cancer.

The effects of smoking on total testosterone levels are contradictory and unclear. Elevated levels of the order of 9-25% have been reported by some studies in middle-aged and older smokers while some other studies have identified an inverse relationship between cigarette smoking and testosterone levels.

The effects of alcohol upon testosterone are dependent upon the pattern and duration of usage. Although alcohol inhibits testosterone production but sustained stable alcohol intake in healthy older men has not been found to influence total testosterone levels.

Physical symptoms of abnormal testosterone levels include poor or no erections, decline in sexual activity, loss of muscle strength, fatigue and loss of energy, sleep disturbances, problems in circulations etc while various mental symptoms include decreased intellectual ability and memory loss.

Each individual may experience a different number and type of symptoms. 'National Silence' On Sexual Behavior, Race, Poverty Contributes To High Rates Of HIV, Other STIs, Opinion Piece Says

The "national silence" on issues such as sexual behavior, race and poverty has contributed to the high rate of HIV and other sexually transmitted infections among teenagers in the U.S., Robert Fullilove, associate dean at Columbia University's Mailman School of Public Health, Adaora Adimora, associate professor of medicine at the University of North Carolina-Chapel Hill, and Peter Leone of the North Carolina Division of Public Health write in a Washington Post opinion piece.

According to the authors, a CDC study released earlier this month that found that about 25% of U.S. girls and young women ages 14 to 19 have at least one of four common STIs is "already old" news for people who work in public health. They add that public health workers "fear this latest study will have its 15 minutes in the spotlight and also fade from view," just like a similar study released 10 years ago by the Institute of Medicine did. The "taboo" of talking about sexual behavior, poverty and race is one "obvious reason" that rates of STIs remain high, the authors write, adding, another is "that the incidence of [STIs], particularly HIV, is concentrated in poor, segregated neighborhoods that are characterized by high rates of incarceration." The "shift" in marriage and courtship patterns that results from men being incarcerated, as well as an increase in the number of "multiple concurrent sexual partnerships," also are contributing to the problem, according to the authors.

HIV/AIDS and other STIs cost the U.S. "tens of billions of dollars" annually, "but with the exception of HIV infection, [STIs] remain the elephant in the room when it comes to the national conversation about health and health care," the authors write. They add, "We can no longer have effective [STI] prevention campaigns in poor communities of color if they treat one person at a time or ignore social conditions underpinning high rates of HIV and other" STIs. "Simply put, we will never rid the U.S. of HIV and other [STIs] if our only weapon is medical treatment," the authors write, concluding, "And if we are unable to engage in a national dialogue about the sexual health of our youths and the social dynamics that drive [STIs], this epidemic will go largely ignored, and many more lives will be lost" (Fullilove et al., Washington Post, 3/21).

PhRMA Spent $22M Lobbying Congress In 2007

The Pharmaceutical Research and Manufacturers of America in 2007 spent more than $22 million on efforts to lobby the federal government, about a 25% increase from the amount spent in 2006, according to disclosure records filed by the group last week, the AP/Houston Chronicle reports.

According to the records, PhRMA spent $12 million in the second half of 2007 to lobby against several proposals, such as a bill that would allow the federal government to negotiate prices directly with pharmaceutical companies under the Medicare prescription drug benefit and legislation that would allow the purchase of lower-cost medications from Canada and other nations. Congress did not pass either of the bills. PhRMA in 2007 also lobbied against a patent reform bill, which remains under consideration in the Senate, and a prescription drug safety bill, which became law last September but did not include restrictions on direct-to-consumer advertisements opposed by the pharmaceutical industry.

Experts said that, although the increase in spending on efforts to lobby the federal government in 2007 appears to have "paid off on some key issues," the "road ahead for the industry looks increasingly bumpy," the AP/Chronicle reports. Massie Ritsch of the Center for Responsive Politics said, "This seems to be a case where the increase in lobbying activity is defensive, not offensive" (Perrone, AP/Houston Chronicle, 2/22).
